Outreach and community services/strength-based support
- Capacity building support exploration of individual goals/interests (choice & control) as per ndis plan
- Accessing community ie taking individual to do leisure activities to promote community engagement ( group activities ).
Supported independent living
- Supporting participants to live independently, in a share accommodation depending on needs ie assistant with budgeting, technology communication purposes and learning new skills.
In home support
- Domestic assistant ie cleaning, home maintenance and around the clock personal care ie medications prompt/medication administering
Support coordination Level 1 & 2
K & M Support Service is working in collaboration with Allied health professionals and have support personnel’s who will work in partnership with our participants to identify and link with the supports you need to live a wholesome and fulfilling life
Respite Care short and medium term
- K & M Support Services offers short and long term Respite and Foster accommodation services to individuals/ children with disability and mental health.
- We also provide short and long term foster care to individuals/ children experiencing domestic violence.
